As a Care Plan Coordinator you will be the first point of contact for service users and their families. You will need to be methodical and organised and have an eye for detail. You will also need to be patient and understanding and be able to prioritise your own work. You will be responsible for carrying out initial visits to service users to discuss their personal care plan and how best we can meet their needs taking into account their wishes wherever possible. You will need to have a good understanding of person centred care and be able to develop care plans which take into account service user wishes whilst ensuring that care is delivered in a safe manner and in accordance with CQC and local authority requirements.
You will also carry out regular service user care plan reviews and will be expected to attend planned social work reviews in the service user’s homes which will require liaison with other health professionals. A typical day in the life of a Care Plan Coordinator will involve contacting new service users and arranging to visit them in their home to introduce Safehands to them and to discuss how we can best deliver the care they require or have been assessed as needing by the local authority. Once the visit is completed, the care plan is typed up and a copy is delivered to the service user’s home. This needs to be done quickly but needs to accurately reflect how the care is to be delivered. You will liaise with the Social Care Coordinators to plan your visits and also the Supervisors to arrange regular visits to review and amend care plans for existing service users. Working alongside the Social Care Coordinators in the office you will be trained to enable you to cover for holidays and sickness of the Social Care Coordinators.
This is an office based, salaried position and you will be working 37.5 hours per week Monday to Friday 9-5 though you may occasionally be expected to cover on call which is out of hours. In exceptional circumstances you may be asked to deliver care to service users for which payment in addition to your salary will be given.
